When the blue car is taking the second exit, this could be considered either left or ahead. Either way, a left signal is given when passing the first exit, not on approach.
For the third exit, the blue car treats it as ahead, even though it is slightly past the “12 o’clock” position. A left signal is given when passing the second exit, but not on approach.
The red car is treating the third exit as a right so the approach position is on the right and a right signal is given on approach, with a left exit signal as usual.
